Outcomes Monitoring

We use a range of tools to measure progress over the term of the intervention. The family and support worker jointly “score” the family at the beginning and end of the intervention. This gives us a measure of progress under the following categories:

  • Promoting good health
  • Meeting emotional needs
  • Keeping your child safe
  • Social networks
  • Supporting learning
  • Setting boundaries
  • Keeping a family routine
  • Housing and finance

We can report to the agency commissioning our services as required. A review at six weeks will detail:

  • All interventions and input completed
  • Progress towards achieving desired outcomes
  • Any new or outstanding needs identified
  • Plan of work for next six weeks

The final report at 12 weeks will detail:

  • Overall progress
  • Qualitative outcomes
  • All interventions and input completed
  • Progress towards achieving desired outcomes
  • Any outstanding or new needs
